  Please submit the following items with your application:

• Log of 50 C4C Visits via Track it Forward
• Certificate of Completion of  “Sharing Hope in Crisis” Billy Graham Online Training
• Complete the introduction along with the first two modules (days 1-10 and days 11-20) of the Introduction to Chaplaincy course through the Christian Leaders Institute. Each module ends with an exam. Please screenshot the final result after you have taken it and include with Chaplain application.
